Patient Safety and Incident Management Workshops
Patients for Patient Safety Canada (PFPSC) has been a key partner in the development of the key patient safety (PS) and incident management (IM) guidelines, capacity building, programs, policies, and practices that are seen across the healthcare in Canada. Building on that, we developed a unique workshop that combines our lived experience with evidence and interactive activities.
The workshop will provide participants with the motivation and resources needed to improve patient safety and respond to incidents in partnership with patients and families. This half-day in-person or 3-hour virtual workshop will add value to anyone interested in and responsible for patient safety and incident management. PFPSC customizes the program for each audience and can adjust the duration of the session if needed.

How things go wrong.
Using an actual case of harm, we will explore what patient safety means to you, why harm happens, how patient and healthcare worker safety are connected, and how to create the conditions for safe care – including effective listening, communication, and speaking up. We will explore what if questions to reflect on safety critical moments that will help you apply what you learn.

What comes after.
Next, we will contrast what happened in the example with what should happen after an incident according to evidence. We will highlight the needs and expectations of patients and families as well as healthcare workers emphasising how all impacted need to be supported in order to heal. By reflecting on what now we will explore incident management in your organization.
Learning with improvement.
Finally, we will discuss the improvements made in the example while reflecting on other ways to improve safety by learning from past harm. We will note why and how patients and families can be partners in safety noting the shared commitments to improve patient safety and quality of care. Reflecting on what next will help you think about what you can do better to improve patient safety.
