This series of four infographics titled “The View from Here” and the accompanying handbook fills a knowledge gap about how to communicate with patients and families after a harmful patient safety incident (PSI) from the point of view of patients and their families. This perspective is crucial in helping healthcare workers and leaders understand what patients and families need when the patient is harmed during a healthcare incident, and how to ensure they are not further harmed by the disclosure and incident management process. The infographics underscore the importance and value of communicating openly and honestly with the patient and family after a PSI. They also explain why the patient and family perspective is key to the investigation of the PSI and to the recommendations for improvement.

Handbook

The purpose of the handbook is to supplement the information provided in the four “The View From Here” infographics. Where the infographics are intended to be used as handouts for patients and families, healthcare leaders, healthcare workers and participants of workshops on patient safety incident management, this handbook is intended for trainers and leaders who wish to expand further on the messages contained in the infographics.
