Canadian Patient Safety Week 2025 (CPSW) occurs between October 27-31/25 and the theme this year is All Voices for Safer Care set by Healthcare Excellence Canada.  This year’s theme aligns well with the Patients for Patient Safety Canada (PFPSC) Vision of ‘Every Patient Safe’. 

The theme, All Voices for Safer Care, invites all Canadians to become involved in actions to ensure that healthcare is safe for everyone. This could be as simple as asking questions about medications you or your family members are taking, being prepared to share information about your health concerns, joining a patient advisory group or writing to your Minister of Health to encourage adequate funding for public healthcare.  Safe healthcare happens when patients, families, caregivers, healthcare providers, health leaders, government officials, healthcare advocacy groups, communities and healthcare industry partners collaborate. 

Below are the thoughts and lived experiences of Patients for Patient Safety Canada members and how, in partnership with healthcare providers, they made a difference in healthcare directly or through their involvement as a patient partner on healthcare committees. Communication where all voices are heard is one of the most important factors in ensuring all patients are safe. There are examples of how providers can encourage feedback/communication with patients / families/caregivers; also how they can maintain, build or regain trust. Together we can elevate patient perspectives of safe healthcare. 

Paula Orecklin provides timely and helpful questions and tips for patients and families to consider about Artificial Intelligence (AI) in healthcare.

Donna Davis highlights the importance of patient/family input in all of their healthcare interactions. As well Donna underlines the critical partnership between patients, families and healthcare providers in ensuring compassionate and truthful conversations which lead to safe healthcare outcomes.

Esha Ray Chaudhuri describes how ‘equity’ provides a critical lens to integrate “All Voices for Safer Care” for every patient everywhere .
